Cumberland County SCHS provides case management to families with children, birth through age 21, with chronic health conditions, disabilities, or developmental delays.  Connects families with resources and services needed for the child. Free and voluntary program.

Special Child Health Services
Special Child Health Services assists in the procurement of coordinated and comprehensive services for children, birth through age 21, who have a chronic health condition, disability or developmental delay. Assistance includes, but is not limited to:

• Informal Counseling & referral
• Assessment of family & child needs
• Assistance with health & educational services
• Crisis resources
• Case management & monitoring

Assists in acquiring the following:

• Early Intervention Services
• PerformCare ID/DD and DDD eligibility
• Supplemental Security Income (SSI)

Eligibility	Families with children, birth through 21, who have a chronic health condition, disability or developmental delay and are in need of services or resources.
